Request More Information flow not working with Document Type block

Summary:

Hi,

I have set up a Request More Information flow with the Document Type block for Licenses and Certificates. When I send the RMI form to the candidates and they open it, the form is empty and the block is not shown. The same thing happens with another RMI that I set up with both the Document Type block (not shown) + the Supporting Documents block (shown).

Does anybody know why this happens and how to solve it?
