Oracle EBS R12 AP AuditTrail Update Tables Fails with Fatal Error in fdasql, fdacv, and fdaupc

I have enabled Audit Trail in Oracle EBS R12 for the Accounts Payable (AP) module. However, when I run the AuditTrail Update Tables concurrent program, it fails with the following errors:

Fatal error in fdasql, quitting...
Fatal error in fdacv, quitting...
commit
Fatal error in fdaupc, quitting...

My questions are:

  1. How can I identify the exact table, audit group, or database object causing the AuditTrail Update Tables concurrent program to fail?
  2. How can I verify whether AP_SYSTEM_PARAMETERS_ALL is registered in another audit group or has an orphaned entry in FND_AUDIT_TABLES or FND_AUDIT_COLUMNS?
  3. What is the recommended and supported approach to resolve this issue without affecting existing audit configurations or historical audit data?
