How to modify notifications to show the delegate that approved on behalf of an Approver?

 We would like to modify the nofication to bring in more details to satisfy audit requirements.  Currently, when an Approver delegates authority, the notification does not provide the name of the delegate who signed on behalf of the Approver, only the Approver.  We would like to modify the notification to show the name of the delegate as well. 

Has anyone made similar modifications?  What is involved?
