Condition Monitor if a Newly established environment is Not creating any Action Lists

 " We have just built a Brand New environment, and now Condition Monitor is not creating any actions at all. We have checked the basic things like the Collection Rules, Assessment Rules and Action Templates and Items being present which violate Conditons defined. Appreciate any idea's??
