Configuring Oracle Applications Forms 11g ESSO-Logon Manager

Friends,
I have a problem when setting the template for an application developed in Oracle Forms 11g.
The wizard does not detect the fields of application as shown in the attached image.

Any suggestions to resolve my case.

I use the version of the ESSO-Logon Manager 11.1.1.2
