Saving response comments following notifications

I have created a custom workflow to send notification requests for approval.  As part of the notification approval / rejection, we want to allow for entering comments along with the approval / rejection response.  I found in the Workflow Developers guide how to create a message attribute of WF_NOTE to prompt for that comment.  How can I get that response value following the notification activity to save in my custom table?  I have been unable to find any information on this, and would appreciate any insight.
