custom resource request

 Hello. 
What we have is a custom resource defined in OIM 11g.

I created the RO, Process Def, Dataset, and attached the adapter to create user task in process def. I also created a request template utilizing the dataset.

However, when I login as a user and request the resource, provide the new user information, and go through the approval (it auto approves right now to make it go quickly), I get this error in the log:
