how to provide form to user during resource request in OIM 11g

Hi Folks,
We have setup OIM 11g and configure OIM workflow to request a resource for a user as explained in OBE http://www.oracle.com/webfolder/technetwork/tutorials/obe/fmw/oim/oim_11g/Custom_Approval_Process_for_Resource_Request/custom_approval_process_for_resource_provision.htm.

we tried to modified the workflow to display form that the requester must fill during the request process by using datasets and export datasets to mds, however during the request process, the datasets or form is not displayed.

Can anybody advise how to configure datasets to display this form? in the past (OIM 9.1) we configure this using Object Form.
