Orchestration in OIM 11g

I am newbie to OIM 11g and would like to share my few doubts with experts here .

In our testing environment , I am seeing a weird inconsistent issue . when we create a user via trusted recon using GTC DB APP table connector (in incremental mode), the user gets created and our own custom event handler like for email generation etc fires and generates and updated th info on user profile but when i see the OOTB display name field , its null (not sure what causes it).

when i go to the DB , i dont see any entry in the Orch* tables for this user . I m sure that event handlers triggered as i can see the email etc populated on the user form . I wondering why i dont see any entry in the ORCH table .
