User got multiple mails for same notification.

Hi All,
One of Very KEY user got 16 mails for same notification, can you please help me how to investigate further.

We have contact SMTP/OS admins, but there is no issue in SMTP/OS. 

SMTP Exchange Server getting multiple relay from application server, for one particular user who got 16 mails.

we have already try to query wf_notifications, wf_notification_out etc tables but no more information.
