OAS SSO 10.1.4.3 blank page redirect

Hi!
I have a problem in SSO 10.1.4.3 and APEX SSO integration based on Note Configuring an APEX Application to Use SSO With SDK in Separate Schema [ID 562807.1].
Test case is following:

1.User navigate to http://mysite.com and redirected to my SSO server -> https://login.mysite.com
2.Enter correct credentials and redirected to apex application successfully, we see new record in ORASSO.WWSSO_AUDIT_LOG_TABLE_T.MAINTAINER_ID -> CRED_LOGIN, Login Successful
4.After logout, user redirected to SSO login page by p_done_url(
