Requisitions erroring out after final approval R12

 Dear Gurus,We have three issues with workflow. 
1. After final approval, requisitions are still stuck with status "In process" when viewed in workflow. This makes it impossible to autocreate POs from such requisitions.(We use forms to raise Requisitions)
2. Invoices sent for approval are not seen by the approver. This happens especially when vacation rules are created.
3. Approvers and requestors do not receive emails notifying them of a transaction in the workflow that requires their attention.

Please kindly assist me with these issues.
