AME vacation rule causes user to approve the requisitions twice

I have 2 items that are causing problems with AME.

I set a vacation rule in one instance to delegate the approval to the manager of the employee.  this causes the manager to approve the document twice

2nd is i set a delegate to a manager of the employee who is on vacations. This send the approval to this manager, but if the approval is over 500 dollars it sends the approval to the CEO of the hospital.  the manager is allowed for approval up to 50,000 dollars

Any one know a setting change that could take place that would fix this.  both items seem to be logically working according to the AME build but for this business instance the logic needs to be changed.,
