Need information on how to current OID/SSO components from Windows 2003 64 bit Windows 2008 64bit?

 We currently have OID/SSO 10.4.1.3 installed on Windows 2003 64 bit machine. The installation is a default installation and the database is the database that comes with the installation on that same server. We need to move all of this to Windows 2008 64 bit and are seeking instructions on how to do this when the IP and server name are quite different.I have not found much information on how to do this. Please help -
