c0000005 EXCEPTION_ACCESS_VIOLATION error on SQL Server 2012 linked server using ORAOLEDB provider

Hello, we are intermittently experiencing the below error using a SQL Server linked server using the Oracle ORAOLEDB data provider.  This is occurring in both SQL Server 2012 and 2008.

*   Exception Address = 0000000077864320 Module(ntdll+0000000000054320)                                         
*   Exception Code    = c0000005 EXCEPTION_ACCESS_VIOLATION                                                     
*   Access Violation occurred reading address 0000000000000000 

Has anyone else seen this?  How did you solve it?
