Attachment View error

We are using File Attach, View & Detach functionality on a custom page.
For Transaction 1 :-
The Attached file name shows up on the page, Attach button is greyed out and View and Detach buttons are enabled.
When I click on View button, I get the error : File Not Found(137,29)
The file is not found or file is empty so the operation could not be completed.
For Transaction 2 :- (and all other transactions)
The Attached file name shows up on the page, Attach button is greyed out and View and Detach buttons are enabled.
When I click on View button the file opens up without any error message.
