11i to R12.1.3 Upgrade

Hi,
Is there a consolidated note id which gives the information on all the pre installation  / pre upgrade patches that are required to applied on an 11i instance at each product level for EBS before proceeding  to apply the main patch for R12 upgrade . Is there such an information for India localization as well.
