No Data Error while attempting to open Collections form from Collections Agent

Hi

I am getting NO Data found error when I attempt to open Collection Agent>Collections form.  All the setups are correct, I dont know why I am getting this error.

Can anyone help me?

ORA-01403: no data found

FRM-40735:Post-Form trigger raised unhandled exception ora-06508

I have recompiled the plls as well the Formx(IEXRCALL.fmx), but still have the issue...

Your help would be appreciated
