Approval e-mails are not being accepted by workflow

We have been utilizing e-mail approval for requisitions for many years (we are now on EBS 12.1.2).

Lately the e-mail approvals have not been working, users are getting a message back stating

Warning: Your previous response to this message was invalid (see error message below). Please resubmit your response.

Error Message: 3003: Lookup code '' is not a valid code for lookup type 'PO_DOC_APPROVE_ACTION_SIMPLE'

Value Found: None 

Remarks: Please use only the suggested values, making sure they are enclosed within quotes (For example "Approve", "Reject", and so on)


This started towards the end of May and was intermittent at first, but now affects all users on all platforms and clients (PC, Mac, iPad, iPhone, Android...). As best we can tell, no changes were made to our operating environment, our Oracle mail servers (which go thru Kodiak), our internal mail server (hosted by Google), or anything else we can identify. On inspection of the approval e-mail, there is the word Approve between the quotes.
