Database level audits for security

We have deployed database level auditing for PSOPRDEFN and PSROLEUSER.  AUDIT_OPRID is not consistently populated with the correct OPRID.  We do have EnableDBMonitoring set to 1 (please do not reply back with document 621733.1).  I've also read that IB and 3rd party updates may not get updated correctly (Doc ID 1137031.1 and Doc ID 1348804.1).  Our security manager is counting on this audit "to ensure integrity of the system".  I've tried recreating the scenario (with no luck) and I'm pretty sure the update is thru PeopleCode.  When we hire or terminate we update.  We are also changing roles if a user signs in outside of the firewall (using sign-on PeopleCode).
