Some Req Approval Emails Not Getting Sent?

I'd like to first apologize for my lack of workflow knowledge - hoping to take a class soon!

We've been having intermittent issues with workflow emails - mostly just those are sent to approvers when they have a new req to approve.  Everything looks ok from the Workflow Status Monitor and the user's settings are all correct.  I was looking through the mailer logs to see if I could find anything and I came across something that was questionable, but I'm not sure if it indicates a problem or not.

The user that claims he's not getting the emails has this in the log below the section where I see him identified...
