approved request and orders are not sending emails to user

We are using EBS version 12.1.3

User noticed that approved request and orders have stopped sending them email notifications. The workflow dashboard shows all components are up. A test to the mailbox itself shows the mailbox is able to send and receive emails. Where should I start to investigate  this issue? Gurus, please advise. Thank you very much.
