Add custom form to RMS

Hi guys,
We want to add a custom form in RMS application and had the form compiled. do you know how to show up the new form in RMS application? my colleague did it in the previous project about how to show up the form (he said there is a menu in the application to control the form) but I haven't found it yet. your help is much appreciated. thanks
