Generic Template - Email Creation

I have created a notification template in response to a user request.

I successfully generate an email when the Notify button is invoked.

The user wants a 'Reply' email notification to be sent to the person who initiated the request.

However, the email cannot be generic.  The user would like to enter additional information if necessary, just like they can when the Notify button is invoked. 

Is this possible to satify the requet of a 'Reply' email using a Generic Template?
