Notify Requester when voucher has match exception of Rule_R500

Has anyone successfully created a sql or query that can identify the Requisition - Requester whenever a voucher is entered and matching generates a match exception for Rule_R500?

Please note I'm looking to identify the user that created the associated Requisition when a Purchase Order is used, NOT the buyer that's responsible for the Purchase Order.

Any help is appreciated.
