workflow error

Hi Experts
I have created  a standard po and I have tested in ame test workbench.It is working fine and showing the correct approval hierarchy and showing first approver being notified. But at the same time when approved by 1st approver/or even when rejects showing following error.
ERROR: ORA-01403: no data found ORA-01403: no data found ORA-01403: no data found ORA-06512: at "APPS.WF_NOTIFICATION", line 5130 ORA-06512: at line 1
