Attachement document showing txt document while approving operations using oracle cMRO

Hello members,

I have configured workflow for operations in oracle cMRO, but while trying to open references "Approval Required Document" it is showing a text document with no information. Can anybody guide me how an approver can see operation details on workflow page? Currently it showing only action history with "Approve", "Reject" "Reassign" and "Request Information" button.
