A lot of messages in wf_notifications with subject - no data found ORA-01403: no data found

Hello,

We have the situation of having a lot of  records with the following subject:

                         Error in Workflow POAPPRV/<item_key> ORA-01403: no data found ORA-01403: no data found

This records are generated for PO's that stuck in status = 'IN PROCESS'.

Could you please suggest the steps of resolving this issue?

EBS: version R 12.1.3
