Custom AWE email approvals

I have a custom AWE that is working today and we are using email approvals.  This all works great!

I am in the process of applying the Enterprise Component patches 1-11 and now in dev, the email approvals don't work for the custom AWE.  What happens is the outgoing email is sent, the user approves it and sends back the approval.  The email is received in the mailbox for processing, but the approval never happens.   This works fine for requisitions and they are getting approved with no issue.

Does anyone have any suggestions on where to look?
