Forms SSO - OID 9.0.4.3 connection problem

We have SSO enabled Forms. The application works since 2006. However, in the past three weeks we face the problem that the SSO application cannot connect to OiD.


1. In <INFRA_HOME>\opmn\logs\OC4J~OC4J_SECURITY~default_island~1, we see

Unable to establish connection to directory.     

2. In <INFRA_HOME>\sso\log, we can see

Communication Exception received. Cleaning up the stale connection

oracle.ldap.util.CommunicationErrorException: Unable to establish connection to directory. Please verify the input parameters: host, port, dn & password Connection reset

3. In <FORMS_HOME>\j2ee\OC4J_BI_Forms\application-deployments\forms90app\OC4J_BI_Forms_default_island_1, there appears
