OAM 11.1.2.1<OAM-02054> <SSO session creation failed> in cluster environment

Dears,

We are facing an issue with our SSO using OAM 11.1.2.1 cluster of 2 nodes.

Sometimes, after working normally for couple of days, when a user tries to login (SSO login), OAM blue error page is displayed, and the following error is thrown in the log:

<Dec 23, 2014 2:03:32 PM EET> <Error> <oracle.oam.controller> <OAM-02054> <SSO session creation failed.

oracle.security.am.common.utilities.exception.AmRuntimeException: Unable to create and persist user session

        at oracle.security.am.engines.sso.adapter.AbstractSessionAdapterImpl.createUpdateSession(AbstractSessionAdapterImpl.java:634)

        at oracle.security.am.engines.enginecontroller.SSOEngineController.createSession(SSOEngineController.java:2426)

        at oracle.security.am.engines.enginecontroller.SSOEngineController.processEvent(SSOEngineController.java:494)
