Notes Creation Error in Collections

Created a personalization in Teleservice which launches Collections.

The personalization works fine. Takes the Account Number from Teleservice, launches Collections with all the tabs and corresponding details populated.

The issue is when I try to create and save a new Note in the Notes tab it displays an error dialog box with no error message.

Does anybody know what this means and how to troubleshoot it?

Does it have anything to do with assigning the correct functions?

Assigned all the relevant IEX functions.
