RMS 13.0.1 Installation Issue

Good Day Guys!

We want to upgrade our current RMS 13.0.1 version to RMS 14.0, so what we did is we clone our production database server to a new server, and we install RMS 13.0.1 application pointing to the cloned server. We have installed the application successfully however we are stuck in the welcome form of Oracle Application form services upon launching the URL provided by the RMS installation. Please see attached file for your reference. What modification we need to do in order to fix the problem. Thanks so much.
