Workflow notification by default goes to cancel state

In the above workflow, sometimes during the approval of the notification, goes into cancellation of the notification and directly goes the node before end and in workflow status, none of the other nodes are seen. Also the workflow is getting completed automatically.

This issue is not consistent.
