Anyone having issue with Bank Statement Workflow not sending notifications?

In our TEST environment I am in the process of reviewing and testing the Procurement Cards module in preparation for our site. We upgraded to FSCM 9.2 in December 2014.  When running the Bank Statement Workflow process (PO_CCSTMT_WF) cardholders are not being notified via email to let them know their billing statements are ready to be reconciled.   I have checked our security setup for card holders which appears okay.  Also, our notification process for requisition approval is working fine notifying approvers via email.  So I'm puzzled as to why Bank Statement Workflow is having issues with notifications.  I'm afraid it will be something real simple that I overlooked. Anyone else experience this problem and how did you correct it?
