oim-ad provisioning fails

we have a newly created  adapter that is failing and stopping users from one of our business units from being automatically provisioned by Oracle IDM.  This is a new Application Instance that was added to Oracle IDM, and the AD domains that were already being provisioned to are still being provisioned correctly.  The users from the new business unit are being stuck in pending state on the ‘System Validation’ step in resource history.  I have checked and made sure that the Auto Save box in the process form is checked.

The adapter in question is the adapter that pre-populates the userprincipalname field.  It combines their user login value with a literal, in this case ‘
