iProc req rejected by system but workflow shows every approver approved

Hi

I have a problem with one of the req and PO. We have a PO that we added extra amount and it went to the approvers. Every approver (5 Levels) approved this particular PO. But the user got a workflow notification email saying that the PO was rejected. How is it possible. I checked the notification details and it says all the 5 approvers approved the PO.

Can anyone help me with this.
