Journal Stucked at Awaiting Further Approvals Status

We have a 3-stage journal approval process. The journal has been fully approved in Stage 1, but the Stage 1 approval status did not change from Awaiting Further Approvals to Approved. Subsequently, the Stage 2 approval did not kick-off as it is waiting for Stage 1 to complete. I have attached a screenshot of the workflow (not sure if it will still be any help after I masked most of the details).

As a workaround we are resubmitting the journals, which goes fine the second time around (i.e. the journals get approved ssuccessfully without getting stucked). However, due to the number of journals  being created / submitted for approval, it is ideal to find out what is causing the issue and the permanent solution.
