OIM Look-up's vanishes automatically

hello guys,

I have added some custom fields in my form and for those fields i have process triggers. Below is the example of the lookup that i have added for one of the custom field.

Ex: Code : "USR_UDF_VF_BYOX_BAN"     Decode : "Change BAN"

Like above i have many fields and triggers but what happens is few of the lookup's like above is getting disappeared(means the lookup is getting removed automatically) after a few hours sometimes after couple of days. I keep adding it once in a while but i do want solve this issue. But the default or system lookups stays there only the custom vanishes. Kindly help me to fix this.
