Amount total does not equal Base Amount total for an account in USD-based business unit

For a US$ based business unit when the trial balance reflects an amount total of US$ 0, and a base amount total of US$ 5.00, how would we correct this to show both amount total US$ 0  and base amount US$ 0 effectively clearing out the balance? We are unable to reproduce how it occurred but are simply looking to resolve the issue.
