Change Assistant Failure: Connection failed - Please check Access id/password.

While trying to run the step "Re-creating Upgrade table", step is failing with the following error message.

Change Assistant Failure: Connection failed - Please check Access id/password.

In order to troubleshoot the issue, I tried re configuring the target environment and it was failing with "File: E:\pt85411c-retail\peopletools\src\psjca\psjsp.cppSQL error. Stmt #: 2457 Error Position: 0 Return: 8010 -

[Microsoft][SQL Server Native Client 11.0][SQL Server]Invalid column name 'GPS'. Failed SQL stmt: select GPS from PS_INSTALLATION"

I verified the INSTALLATION table and noticed it is having 152 columns for the record on the app designer and having 183 columns on the database. Not able to proceed with the upgrade.
