Fed: Duplication of Treasury Confirm

Hello

We are currently on R12.1.3 version, and are running in to a issue with duplication of treasury confirmation of a payment batch. It is very random and cannot be "replicated" at will.  I think this may also have been happening in 11i, but in that environment, user was able to back out the confirmation and both confirms would back out. but in R12 it does not seem to do that. It creates 2 confirms, and when we back out it only backs out 1. Has anyone else seen the same issue?
