9.2 HCM Pagelet Issue

We went live on 9.2 HCM, tools 8.54.10, PUM 12. We have a very strange issue in our Prod environment:

Delivered pagelet, BN_HEALTH_BENEFITS,  works fine in DEMO, DEV, TEST, INTG environments. This pagelet does not work in PROD, in the sense, the source (PS_QUERY) gets the data, however, system is unable to render the 3D chart (we see a tiny black square box with X)

This pagelet has never ever worked in PROD (as went live recently). We did not change the pagelet and it is a delivered one.

Can anyone provide any options to fix the issue?
