GL Journal Approval Workflow SQL Error

Need help determining the source of this SQL error.  So far it cannot be recreated on command, but does pop occasionally.  We get this when attempting to submit a journal for approval after it has been validated.  Sometimes after you get the error you can logout and back in and be able to submit, other times you cannot.  So far nothing consistent enough to trace from our side so I am reaching out, hoping that someone has experienced this before.
