Post upgrade from 11i to R12.2.5, an error appears on the Processes Folder

We are in the process of upgrading from 11i to R12.2.5.

While testing a functionalities, we encountered this error:

Step by Step process:

1. Login to GL Responsibility

while on Forms home page, there are three tabs appearing

1. Functions

2. Documents

3. Processes

Normally, if we click the Processes folder, it should show us the workflow process that would be running at the back-ground. However, the below error is appearing while clicking "Processes" folder

Your userarea applet has caused a runtime exception, the applet will be

terminated to ensure continued functioning of your form.
