OIM 11g R2 Approvals

Hi,

We are facing an issue in beneficiary manager approval.

Once the assigned request is approved the task is showing completed. However, the request status is still showing obtaining operational/Request Approval accordingly and the account is not getting provisioned.

Please some one help me it is urgent.
