Dunning Template issue

We are trying to create a custom Dunning Template as we need to add in the customer's PO # and sales order number to the dunning letter.  However, after we run the Dunning process, the Dunning letter does not have any output.  We have errors in the log:

IEXXmlDelivery [May-13-2016 10:12:27]: updateStatus: sTempStatus=XML TEMPL PRCS FAILED DUN_COPY

IEXXmlDelivery [May-13-2016 10:12:27]:Failure Reason: Failed to submit delivery request. Check the corresponding profile values and config file
