Duplicate column in a grid FSCM 9.2/ Tools 8.55.04

We are upgrading from FSCM 9.1/8.51.21 to FSCM 9.2/ 8.55.04 and running into a strange problem with Auto Reconciliation page in 9.2 after upgrade. We are applied all PUM 18 updates. We see the select column twice for one specific user. For all other users it display just fine. It even displayed fine for the user who was created as clone of the user with display issues. This user with display problems cannot see the exceptions column and this select column appears twice. So far we have cleared cache, run version app engine in enhanced mode and tried to trace
