Wrong files for FIN 9.1 to 9.2 using new update source image 19

I just started using the new Source Image 19 there are a few major issues.

1) Folder names are correct with 8.55.05 but when it installs it used the 8.55.03 version.

2) When configuring change assistant it also thinks the new image is 8.55.03.

3) When viewing information on PIA it shows image 11 not image 19.

I opened up an SR but hopping Oracle can comment on this we are already behind on this upgrade and ever day counts.
