PUM - Change Assistant best practices questions

1.  What is the difference in Initial Pass - Target Steps only and MTP if you do not create a custom change package?  Since we do not create a custom change package, is there any reason we should not run the Initial Pass - target steps only to our final 2 environments (user testing and PROD)?

We are on HCM 9.2 PeopleTools 8.54.15 and have applied updates from PUM 17 to our 2 initial environments (one vanilla and one development) selecting Initial Pass - all steps.  We have 2 remaining environments to apply the updates to - User testing and production. 

2.  Is it a best practice to use the PUM Hub or the Hub from one of our target environments?
